Crisis Prevention for Seniors with Alzheimer's and Associated Dementias is a SMB Partnershp Program put in place with our San Diego partner. It is a dual-purpose program for health and benefits planning, offering an innovative solution to Alzheimer’s and dementia (offered only in San Diego).

Services come in two parts:

1. Health planning

• Treatment options for cognitively impaired patients
• Customized techniques for successful patient care giving
• Direction and support navigating the medical system and accessing community resources

The program begins November 1, 2009.

Currently the program is being in pilot phase, for select enrollees.

The target audience includes adult children, spouses and/or the responsible party for cognitively impaired patients.
 

This program is important in looking for innovative ways to reach SMB's core clientele of seniors and elders with debilitating illnesses.

One of the consistent areas where there is a systemic lack of options for care and funding options are those suffering from Alzheimer’s and Dementia. So often these stricken individuals need help in multiple areas at the same time: health and lifestyle issues, care giving issues, funding problems, lack of benefits problems, long term family strife and destruction of relationships.
